Consent Form for Children under 18 Parent or Guardian,You are allowing your child to participate in a cooking class held at Jeremiahs Cooking Studio that will involve preparing and tasting recipes, as well as using common kitchen equipment. – Classes are designed for the anticipated skill level of designated ages. Please note that your child may be participating in the following tasks, under direct supervision of culinary team members: Ages 5-10: Mix, measure, stir, use of the oven, Light use of stove. Ages 11-17 Cutting/Chopping, Using a stove top and/or oven, Using electric equipment such as food processors, mixers, and blenders – RULES OF THE KITCHEN: 1. NO running, chasing or playing around will be allowed in or on the premise, please explain to your children that it is a kitchen just like home. Knives and other equipment can be dangerous if not used properly. 2. sanitation is key to a great experience cooking. Please make sure that children come in proper clothing, shoes and with hair pulled back. If your child is sick, for the safety of the other children please do not participate. -There may also be photographs/videos taken that may be used to illustrate some of the activities Jeremiahs cooking studio offers for children. Participation in a cooking class automatically includes permission for Jeremiahs to use the photos/videos. IF YOUR CHILD HAS, or YOU THINK YOUR CHILD MAY HAVE, ANY TYPE OF FOOD ALLERGY or SENSITIVITY, it is YOUR responsibility to make that clearly known to the instructor.”


Consent Form for ADULTS

You are agreeing to participate in a cooking class held at Jeremiah’s Cooking Studio that will involve preparing and tasting recipes, as well as using common kitchen equipment.

-Classes are designed for the anticipated skill levels. Please note that some of the class will be a demonstration and some will be hands-on preparation under direct supervision of culinary team members:
Some preparations and equipment you will be using are: Mixing, measuring, stirring, Cutting/Chopping, Using a stove top and/or oven, Using electric equipment such as food processors, mixers, and blenders

– RULES OF THE KITCHEN: 1. Sanitation is key to a great experience cooking, You should be dressed comfortably but with safety in mind. Flip-flops and open-toed shoes are not permitted in the kitchen. Do not wear loose clothing that could get caught in machinery or equipment. Jewelry should not be worn, but if you choose to it is your choice. You should wear hair clips or bands for long hair. If you are sick, for the safety of the others please do not participate.

-There may also be photographs/videos taken that may be used to illustrate some of the activities Jeremiahs cooking studio offers. Participation in a cooking class automatically includes permission for Jeremiahs to use the photos/videos.

-Alcohol/Liquor policy: I understand that Jeremiahs catering and cooking studio does not serve alcoholic beverages but individuals may bring in their own and serve themselves. I agree that if I decide to drink alcoholic beverages while cooking and on the premise of Jeremiahs, that I am responsible for myself and agree to any consequence of my actions and will hold them free of any legal actions. I also agree that I will not operate a motor vehicle after consuming alcoholic beverages during the cooking class.

IF YOU HAVE, or YOU THINK YOU MAY HAVE, ANY TYPE OF FOOD ALLERGY or SENSITIVITY, it is YOUR responsibility to make that clearly known to the instructor.


Cancellation Policy
Seats in a cooking class are much like tickets to the theater. If you are unable to attend, it is best to send someone in your place.   Because we purchase and prep food specifically for the students in the class, we require full payment for classes and camps at time of registration. 

For classes and camps, if you cancel at least 7 days prior to the start of class or camp your payment will be refunded.  If you cancel for any reason within 7 days of the start of class/camp your payment is forfeited unless another student takes the spot.  We are not able to offer credit, transfers or refunds for last-minute illnesses or work-related conflicts, personal emergencies or events beyond our control. However, you are always welcome to send someone in your place, because we understand these things do happen.

Birthday Parties and Private Parties can be rescheduled up to 7 days in advance. Deposits for private parties and birthday parties are non-refundable.  

Class may be cancelled based on low enrollment.  In this case you will be contacted in advance and reimbursed.  

If class is cancelled due to weather conditions, Jeremiahs Catering & Cooking Studio will post notice on Facebook as well as attempt to email or call class participants.  Always call if in doubt!  If class is cancelled due to weather, a reschedule date will be available, if you are unable to make the reschedule date, a refund will be issued.
